ABSTRACT

RESUMEN Objetivo. Determinar el efecto de la ablación folicular en el inicio de un protocolo de superovulación (SPO) sobre la respuesta superovulatoria en vacas donantes de raza Brahman. Materiales y métodos. Se utilizaron 20 vacas de raza Brahman, las cuales fueron distribuídas aleatoriamente en dos grupos: Grupo control (G1; n = 10), la sincronización de la onda de crecimiento folicular fue realizada mediante la combinación de estrógenos (2.5 mg, Benzoato de Estradiol) y progestágenos (1 gr, implante intravaginal); cuatro días después se inició el protocolo de SPO con la hormona folículoestimulante porcina (FSHp); y grupo ablación (G2; n = 10), se realizó la ablación folicular y un día después se inició el tratamiento de SPO con FSHp . En los dos grupos la colecta de los embriones se realizó siete días después de la primera inseminación artificial. Resultados. El G2 presentó una mayor proporción de embriones de calidad 1 (p<0.01) en comparación con el G1 (68.60%, 31.22%), mientras que los animales del grupo G1 presentaron una mayor proporción de embriones de calidad 2 (43.04%, 18.60%, p<0.01). Para las variables total de estructuras colectadas, y total de embriones transferibles, no se observaron diferencias significativas (p>0.05). Conclusiones. La ablación folicular aumentó el porcentaje de embriones de calidad 1, sugiriendo que la implementación de esta técnica, como estrategia para sincronizar el inicio de una nueva onda de crecimiento folicular en tratamientos de SPO, mejora la calidad de los embriones producidos en vacas donadoras Brahman.


ABSTRACT Objective. The objective of this study was to determine the effect of follicular ablation at the beginning of a superovulation protocol (SOP) on the superovulatory response of Brahman donor cows. Materials and methods. Twenty Brahman cows were used, randomly distributed in two groups: control group (G1, n = 10), synchronization of the follicular growth wave was performed by the combination of estrogens (2.5 mg, estradiol benzoate) and progestagens (1 gr intravaginal implant); four days after starting the SOP with porcine follicle stimulating hormone (FSHp); and the ablation group (G2, n = 10), follicular ablation was performed and one day after, the SOP treatment with FSHp was initiated. In both groups, embryo collection was performed seven days after the first artificial insemination. Results. The G2 had a higher proportion of quality 1 embryos (p<0.01) compared to G1 (68.60% vs. 31.22%), while animals of G1 group had a higher proportion of quality 2 embryos (43.04% vs. 18.60%, p<0.01). For the total of structures collected and the total of transferable embryos, no significant differences were observed (p>0.05). Conclusions. Follicular ablation increased the percentage of quality 1 embryos, suggesting that the implementation of this technique, as a strategy to synchronize the beginning of a new wave of follicular growth when using SOP, improve embryo quality in Brahman donor cows.

ABSTRACT

OBJECTIVE: To explore the treatment strategy for patients with hyperresponsiveness in the early and middle stages of superovulation induction(COH)by GnRH agonist down-regulation protocol in in vitro fertilization-embryo transfer(IVF-ET).METHODS: An analysis was made in 127 cases of IVF-ET performed in Reproductive Center of Shantou Central Hospital from June 2017 to June 2018.The patients were divided into 3 groups according to the different treatment:intermittent drug withdrawal group(group A),small follicle aspiration group(group B),and control group(group C).The embryo quality,pregnancy outcome,incidence of ovarian hyperstimulation syndrome(OHSS)and the number of cancelled fresh transplantation cycles were compared among the three groups.The embryo quality and pregnancy outcome was further compared between discontinuous withdrawal once(A1 group)and 2-3 times(A2 group),and between 1 day(A3 group)and 2-3 days(A4 group).RESULTS: The E2 level on HCG day,the rate of moderate and severe OHSS,cancellation rate and miscarriage rate in group A and group B were significantly lower than those in group C(P0.05).There was no significant difference in embryo quality or clinical outcomes between group A1 and A2,and between group A3 and A4(P>0.05).CONCLUSION: The intermittent withdrawal regimen is non-invasive,safe and simple,and can improve the quality of embryos.It is a suitable regimen for high-response patients to improve the chance of fresh transplantation.

ABSTRACT

Objective To compare the embryo development and pregnancy outcomes of patients by using two different 17?gauge aspiration needles in transvaginal ultra?sound guided oocyte retrieval. Methods A retrospective study was performed to analysis the embryo development and pregnancy outcomes of patients by using different oocytes aspiration needle:import needle(group A)and domestic needle(group B),including puncture follicles,oocytes rate,cleavage,available embryos,high?quality embryo,the total fertility rate,bleeding, moderately severe OHSS incidence,pregnancy rate,embryo implantation rate and abortion rate,etc. Results There was no significant statistical difference in the two different ovum aspiration needles. Conclusion The embryo development and pregnancy outcomes were similar with both needles. Each needle has its advantages and flaws in transvaginal ultra?sound guided oocyte retrieval ,and we should make a comprehensive choice weighing the practical situation of patients,cost,effectiveness and the doctors' preferences.

ABSTRACT

The aim of this work is study the laparoscopic ovum pick-up (LapOPU) technique in spotted paca, describing surgery details, complications and oocyte recovery rate. Nine healthy adult non-pregnant captive females were used, in a total of 39 procedures. When the surgical plane of anaesthesia was achieved, the females were positioned at 20º Trendelenburg. Three 6mm trocars were placed on right and left inguinal and hypogastric regions. Abdomen was inflated with CO2 and the intra-abdominal pressure was stablished in 10mmHg. Follicular punctures were performed moving the ovaries with atraumatic forceps. For punctures, an 18-gauge 3.5 inch long needle attached to a vacuum system with pressure not exceeding 65mmHg was used. Oocytes were recovered into 50mL centrifuge tubes with media composed of PBS supplemented with 10 IU/mL of heparin and kept at 36°C. R Software was used for statistical analysis. Data normality distribution (Shapiro test) and variances homoscedasticity (Bartlett test) were tested and descriptive statistics (mean±SD) was used to present the results. It was only possible to perform LapOPU in 30 of 39 laparoscopies (76.92%). The surgical total time was 37.34 ± 18.53 minutes. The total number of visualized follicles, aspirated follicles, and retrieved oocytes were 502, 415, and 155, respectively. And the same parameters per animal were: 14.34 ± 12.23, 11.86 ± 10.03, and 4.43 ± 4.69 respectively. Oocyte recovery rate was 32.56 ± 27.32%. In conclusion, caudal positioning of portals with slight triangulation allows good viewing of the abdominal cavity and eases the manipulation of the ovaries. Thus this described LapOPU technique is feasible in spotted paca and easy to perform.(AU)


Objetiva-se, com este trabalho, estudar a técnica de aspiração folicular por videolaparoscopia (LapOPU) em pacas, descrevendo detalhes do procedimento cirúrgico, complicações e taxa de recuperação oocitária. Para isso, foram utilizadas nove fêmeas, saudáveis, adultas, não gestantes e mantidas em cativeiro, totalizando 39 procedimentos. Quandoo plano anestésico cirúrgico foi alcançado, as fêmeas foram posicionadas em Trendelenburg com 20º de angulação. Três trocáteres foram colocados nas regiões inguinais direita e esquerda e hipogástrica. O Abdômen foi insuflado com CO2, e a pressão intra-abdominal foi mantida em 10mmHg. Punções foliculares foram realizadas manipulando-se os ovários com pinças atraumáticas. Para aspirações foliculares, usou-se agulha de 18G com bisel curto acoplado ao sistema de vácuo com pressão não excedendo 65mmHg. Oócitos foram recuperados em tubos de centrifugação de 50mL contendo meio composto de PBS suplementado com 10UI/mL de heparina e mantidos a 36ºC. Usou-se software R para análise estatística. Testaram-se a distribuição normal dos dados (teste de Shapiro) e a homocedasticidade das variâncias (teste de Barlett) e se usaram estatísticas descritivas (média±DP) para apresentar os resultados. Das 39 videolaparoscopias, só foi possível realizar LapOPUem 30 delas (76,92%). O tempo cirúrgico total das LapOPU foi de 37,34 ± 18,53 minutos. Os números totais de folículos visualizados, folículos aspirados e oócitos recuperados foram: 502, 415 e 155, respectivamente. E os mesmos parâmetros por animal foram: 14,34 ± 12,23, 11,86 ± 10,03 e 4,43 ± 4,69, respectivamente. A taxa de recuperação foi de 32,56 ± 27,32%. Assim, conclui-se que o posicionamento caudal de portais, com ligeira triangulação, permite uma boa visualização da cavidade abdominal e facilita a manipulação dos ovários, sendo essa técnica de LapOPU viável em pacas e de fácil execução.(AU)

ABSTRACT

OBJETIVO: Analizar las técnicas anestésicas utilizadas en el centro de reproducción asistida avanzada Embriovid para la aspiración folicular transvaginal guiada por ecografía (AFT) con el propósito de estandarizar las dosis de anestésicos ,revisar sus complicaciones y el perfil de recuperación en pacientes donantes y en tratamiento de fertilidad. DISEÑO METODOLÓGICO: Serie de casos. Población y temporalidad: pacientes sometidas a AFT bajo anestesia en el centro de alta especialidad de reproducción asistida Embriovid en la ciudad de La Paz- Bolivia en el periodo de Septiembre 2015 a Abril 2016. MATERIAL Y MÉTODO: Se revisaron los expedientes clínicos de pacientes sometidas a anestesia para AFT y se registraron variables biodemograficas, los datos de recuperación y complicaciones. Las pacientes recibieron anestesia total intravenosa TIVA en bolos con propofol y fentanil o en su defecto sedación consiente con fentanil y midazolam o dosis bajas de propofol. Se registraron los datos de la hoja anestésica con dosis totales, tiempos quirúrgicos y anestésicos, y medicamentos no anestésicos administrados. RESULTADOS: Se incluyeron 209 procedimientos divididos en dos grupos de registro, grupo 1 de donantes con 91 aspiraciones (43.5%) y grupo 2 con 118 aspiraciones (56.5%) de pacientes en tratamiento de fertilidad (óvulos propios). 199 pacientes (94.7%) recibieron anestesia TIVA en bolos y 10 pacientes sedación consiente (5.3%). La técnica TIVA combino fentanil con propofol en bolos. La dosis de fentanil fue similar en ambos grupos (promedio 0.1mg) mientras que la dosis de propofol fue mayor en el grupo 1(200mg vs. 180mg) .La técnica TIVA presento periodos de apnea cortos que no requirió maniobras invasivas. El dolor postoperatorio fue frecuente pero de baja intensidad. El tiempo de estadía de las pacientes conservo su carácter ambulatorio. CONCLUSIONES: La técnica TIVA en bolos que combina propofol con fentanil parece ser una técnica anestésica segura en manos de un anestesiólogo y adecuada para la aspiración transvaginal de folículos ováricos.


OBJECTIVE: To analyze the anesthetic techniques used in the center of advanced assisted reproduction Embriovid for transvaginal follicular aspiration guided by ultrasound (TFA) in order to standardize the dose of anesthetic, review it's complications and recovery profile in donor patients in fertility treatment. METHODOLOGICAL DESIGN: Case series. Population and temporality: Patients under anesthesia for TFA in the center of highly specialized assisted reproduction "Embriovid" in the city of La Paz, Bolivia in the period September 2015 to April 2016. MATERIAL AND METHODS: Clinical records of patients undergoing anesthesia for TFA, their biodemographic variables, data of recovery and complications were recorded and reviewed. Patients received total intravenous anesthesia (TIVA) with with bolus of propofol and fentanyl or conscious sedation with fentanyl and midazolam or low doses of propofol. We record the anesthetic dose, surgical and anesthesia times, and no anesthetic drugs administered. RESULTS: 209 procedures divided into two groups registration, group 1 donors aspirations 91 (43.5%) and Group 2 with 118 aspirations (56.5%) of patients in fertility treatment (own oocytes) were included. 199 patients (94.7%) received bolus TIVA anesthesia and conscious sedation in 10 patients (5.3%). TIVA technique combine with propofol bolus fentanyl. The dose of fentanyl was similar in both (average 0.1mg) groups while propofol dose was higher in Group 1 TIVA (200mg vs. 180mg). The TIVA technique shows periods of apnea not requiring invasive procedures. Postoperative pain was frequent but low intensity. The length of stay of patients retained its outpatient basis. CONCLUSIONS: TIVA bowling technique combining propofol with fentanyl appears to be a safe anesthetic technique in the hands of an anesthesiologist and suitable for transvaginal aspiration of ovarian follicles.

A partir de los registros de transferencia de embriones producidos in vitro de tantos años de la central genética en el municipio de San Pedro de los Milagros Antioquia – Colombia, se buscó retrospectivamente si existía unarelación sobre la cantidad de ovocitos y embriones viables producidos por las donadoras. Dependiendo de la raza,se buscó determinar si existe una influencia de la donante. Las variables analizadas fueron: el toro, la raza, el lote, la historia reproductiva de las donadoras, las cuales se encontraban en iguales condiciones de manejo, tanto sanitarias y nutricionales. La evaluación de los resultados se realizó mediante el modelo estadístico de Chi-cuadrado.Para la comparación entre el porcentaje de ovocitos viables y el porcentaje de embriones producidos se realizó unaprueba de análisis de varianza (ANOVA) y una prueba de Fisher con un intervalo de confianza del 95% y un valorde p>0.05. Se encontraron diferencias estadísticamente significativas en la producción de embriones (40% vs 29%), taurus e indicus, respectivamente. Se concluye, que las donadoras taurus producen mayor número de embrionesque las indicus.


From the many years of transfer records of embryos produced in vitro at the genetics center in the municipality ofSan Pedro de los Milagros, Antioquia, Colombia, we examined retrospectively if there was a relationship betweenthe number of viable oocytes and embryos produced by the donors. Depending on the race, we sought to determine whether there is an influence by the donor. The variables analyzed were: the bull, race, batch, and reproductive history of the donors, all of which were in similar health and nutritional conditions. The evaluation of the results was carried out with the Chi- Square test statistical model. In order to compare between the percentage of viable oocytes and the percentage of embryos, an analysis of variance (ANOVA) was performed as well as a Fischer test, with a confidence interval of 95% and a p-value of p>0.05. Statistically significant differences were found in the production of embryos (40% vs 29%) taurus and indicus, respectively. The conclusion is that taurus donors produce greater numbers of embryos than indicus donors.


A partir dos registros de transferência de embriões produzidos in vitro nos muitos anos da central genética nomunicípio de San Pedro de los Milagros, Antioquia- Colômbia procuramos retrospectivamente se há uma relaçãoentre a quantidade do número de oócitos viáveis e embriões produzidos por doadores. Dependendo da raça,buscou-se determinar se há uma influência do doador. As variáveis analisadas foram: o touro, a raça, o lote, a história reprodutiva dos doadores, os quais encontravam-se nas mesmas condições de manejo, saúde e nutrição. A avaliação dos resultados foi realizada utilizando o modelo de teste estatístico Chi-quadrado. Para a comparação entrea porcentagem de oócitos viáveis e a porcentagem de embriões produzidos foram realizados um teste pela análisede variância (ANOVA) e um teste de Fisher, com intervalo de confiança de 95% e um valor de p>0.05. Foramencontradas diferenças estatisticamente significativas na produção de embriões (40% vs 29%), taurus e indicus,respectivamente. A conclusão é que os doadores taurus produzem um número maior de embriões que os doadoresindicus.
